Balneotherapy and venous insufficiency : how water relieves your heavy legs

Can balneotherapy really relieve heavy legs related to venous insufficiency? Yes — and more and more patients are experiencing it. In a bath, shower or during a spa treatment, water acts as a real therapeutic tool on venous circulation. Here's how.

Venous insufficiency in a nutshell

In our legs, blood must rise to the heart against gravity, thanks to small valves that act as non-return valves. When these valves malfunction, the blood stagnates, the pressure rises, and the vein walls weaken(1,2). Result: heavy legs, swelling, pain, cramps and sometimes varicose veins(1,2). What is balneotherapy(3)?

Balneotherapy uses water — its temperature, pressure, movement — for therapeutic purposes.

In the scientific literature, there is no single, rigid protocol: different professionals use different types of care, often combined with each other. In practice, a balneotherapy program may include one or more of the following treatments:

  • Hot Tub: Automatic air and water massage, which stimulates skin circulation and promotes muscle relaxation.
  • Walking in semi-deep water: walking against the current to increase venous return by working the calf muscles, or on a mat of small air bubbles to stimulate microcirculation and proprioception.
  • Balance therapy on irregular inclined surface : stimulates the arch of the foot and venous pump, improves ankle mobility and venous pumping.
  • Bath with massage jets: powerful jets targeted at the lower limbs to drain the tissues and relieve pain.
  • Shower massage: performed by a physiotherapist, from the bottom to the top of the limb, to boost venous circulation.
  • Massage with submerged limbs: gentle manual drainage by a physiotherapist, which promotes the evacuation of excess fluids.
  • Thermal mud application: hot poultices to relieve pain and relax muscles, or cold compresses for skin problems.
  • Deep-water gymnastics: adapted mobilization exercises to strengthen muscles and improve circulation without joint constraints.

Relief of heavy legs with balneotherapy: how does it work?

Water pressure: natural compression(3)

When you dip your legs in water, something remarkable happens: the diameter of veins — including varicose veins — shrinks, blood flow accelerates, and reflux decreases. Water exerts uniform pressure on the tissues, like compression, but in a natural and enveloping way.

The heat and buoyancy of the water act in parallel on pain: they stimulate the sensory receptors in the skin (thermal receptors and mechanoreceptors), which helps to block pain signals. The physical and mental relaxation induced by the bath further reinforces this analgesic effect.

Underwater massages and alternating hot/cold showers go further by stimulating the response of the skin vessels, which contract and dilate in a rhythmic way — a real movement of the vascular system. Finally, aquatic exercises such as swimming pool walk strengthen the calf muscles, which play a crucial role in venous return: by contracting, they propel the blood upwards like a pump. A well-muscled calf is a more efficient venous pump — with more blood ejected with each step and less residual blood stagnating in the veins.

Hot/cold: vascular gymnastics

The warm water dilates the vessels and relaxes the muscles. Cold water tightens them and tones the veins. By alternating the two, we create a real "vascular gymnastics" that boosts the blood return to the heart — hence the importance of finishing with a jet of cold water on the legs(3).

Measurable effects over time

Studies have shown that a spa treatment improves the symptoms of venous insufficiency — heavy legs, swelling, discomfort — maintained up to 1 year after the treatment(3,4). It also improves the quality of life for patients with chronic venous disease when used as an adjunct to usual medical care. These effects justify its use in patients with advanced chronic venous insufficiency who cannot benefit from surgical or endovascular treatments. Balneotherapy, compared to no treatment, can slightly reduce changes in skin pigmentation.

Note: The available studies still have a limited level of evidence; these results are encouraging but should be interpreted with caution.

Balneotherapy at home: the right gestures against heavy legs

To benefit from water on a daily basis:

  • Alternating showering: finish with cold water on the legs, going up from the feet to the thighs — simple and effective.
  • The hot/cold foot bath: alternate two basins (hot and cold), 3 minutes each, ending with the cold.
  • Walking in a pool: in cool water, it is one of the best exercises to strengthen the calves and stimulate venous circulation.

To avoid: very hot baths, sauna and hammam. The intense heat dilates the veins excessively and aggravates the heavy legs.

Precautions and limitations

Balneotherapy can relieve, but does not cure, venous insufficiency. It is always practiced in addition to medical treatment (compression stockings, medication). It is not recommended in cases of open wounds, heart failure, skin infection or uncontrolled hypertension. In any case, seek the advice of your doctor before starting.
